i will go into a brief description of what it's composed of, though. First, two screenshots from my current UI, still pending some possible changes. This is an altered version that I've been working on for the past couple of days in my spare time.

MODS USED
The unit frames are Discord Unit Frames.
much of the UI revolves around this mod. All of it is done with the default package Discord gives you, with some custom fonts i installed and 1 custom texture that i made for the health bar just recently. This was also the hardest to get used to at first. It took me around 2 hours trying to set it up when i first got it, but now, creating anything you want is pretty much a breeze. just mess around with it and see what all the settings do and how it works. my advice is to start with your own "Self" unit frame, and build a simple one from the ground up (IE name, lvl, class, health bar, mana bar). hide all backgrounds and such just to get used to the basics.
The rounded rectangle you see that acts as a border around each unit frame (target, target of target, etc) is an empty discord textbox with a space " " entered in the text field. then u apply background color/settings and border styles, then adjust the width/height of the box to fit your frame. (see Fubar)
Adapt is used for rendering 3d character portraits. this is completely compatible with DUF.
FuBar is used for that top info bar, much like Titan. What i did with that is i made the background transparent, then used Discord Unit Frames to create an empty text box, just as i did with the unit frames, and wrapped it around the information that Fubar provides. Props to Annovec on Skullcrusher for the original idea

Clearfont is used to change all the fonts in the game. Read the README when u download the mod for a link to the original fonts that they are not allowed to include in their downloadable package.
CUSTOM FONTS
Enigmatic Regular (TrueType) used for health percent. im pretty sure this font comes with SCT2
Bank Gothic Light BT (TrueType) used for my name, the target's name, and target of target's name. This is a recent change. the previous font in previous screenshots was Neverwinter Normal (TrueType)
Zrnic (TrueType) used for lvl/class you see in the target and target of target frames.
Calibri (OpenType) used for hp values. This is the original font used in the Clearfont mod, u can probably find a link in the readme.
Custom Fonts used in Discord must be copied into the CustomFonts folder in interface/addons/discordunitframes. you can rename them to simple names easy to retype in the configuration settings when u load up /duf. ie: enigma.ttf, coolfont.ttf, etc.
On a Side Note:
this UI was built for me, not as a general UI package for people, AS SUCH, I encourage you to feel free to play around and think about what you need from a UI. Personally, when i configure my UI, i think about what i need in raids and what i need in pvp. Everything is developed around this. As a tank, i need a big health bar so its easy to recognize how my heals are coming for when i need to hit lifegiving or a pot or shield wall. my target's health is important as well, and equally as important are his debuffs (sunders, etc). less important are my target's buffs, which influences the size at which i scale each to. I need a target of target frame to see who the mob is attacking, and that target of target needs to show debuffs, for something like 4 horsemen. I've grown used to looking at the bottom for my health and at the top for my target and his health, so thats why those frames are there. I also don't need to see my party's buffs, as the only buff im only responsible for is keeping battle shout up on my own group, and so in my party frames, buffs are hidden.
my point is that once you get comfortable working with discord, think about what you really need from your UI and where you want to look for it (or maybe where ur used to looking for it). some information plainly just doesnt need to be there, so save yourself the eye stress and completely hide it. maybe your own HP isnt as important as your parties HP or the target of target's HP. configure your UI as such. Think about what you need, what you don't, what's essential, and what isn't.
A UI, in the end, is more about enchancing your gameplay, and much less about aesthetics.
